📋 Description
• Provide coaching, care coordination, referral and appointing services via phone, text, and video. • Assess needs of participating clients. • Track and document all coaching sessions and related services. • Provide web-assisted audio or video coaching. • Work with clients to develop personal goal plans. • Complete ongoing training as required.
🎯 Requirements
• Provide coaching, care coordination, referral and appointing services to clients enrolled in the VA Caregiver Legal and Financial program. • Assess needs of participating clients to inform the development of personal plans. • Track, monitor, and document all coaching sessions and related services. • Provide web-assisted audio or video coaching. • Work with clients to develop personal goal plans and identify actions steps. • Complete ongoing training as required. • NACCC CCC, Credit Counselor Certification • CFC, Financial Counselor Certification • CFHC, Financial Health Counselor Certification • AFCPE AFC, Accredited Financial Counselor Certification • Associate degree or equivalent experience.
