📋 Description

• Conduct monthly Chronic Care Management (CCM) calls with patients • Support patients with chronic conditions between provider visits • Educate patients on their health conditions and care plans • Coordinate care with providers, pharmacies, caregivers, and community resources • Document patient interactions, update care plans, identify gaps in care, and connect patients with available resources • Spend the majority of your shift making and receiving patient calls while providing personalized support and care coordination

🎯 Requirements

• Active Certified or Registered Medical Assistant credential required • Fluent in English and Spanish preferred (verbal and written communication) • Minimum 1 year of Medical Assistant experience • Strong communication, documentation, and critical thinking skills • Experience with EMR/EHR systems, Microsoft Office, and Google Workspace preferred • Care coordination, case management, call center, triage, or geriatric experience is a plus
