• Responsible for providing proactive intervention and coordination of care to eligible Vaya Health members. • Works with multidisciplinary teams to identify and alleviate inappropriate levels of care or care gaps. • Utilizes licensed clinical knowledge and skills to assess needs and inform care planning. • Supports clinical transition planning and tracks individuals discharged from facilities. • Participates in care team meetings and transitions management.
🎯 Requirements
• Master’s degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area. • Two years of experience working directly with individuals with BH conditions or LTSS needs. • Valid licensure required. • Acceptable licenses include LCSW, LCMHC, and others as specified.

• Must live in or near Rowan County, North Carolina. • Position requires travel. • Requires in-person contacts with members and their care teams.
