• Provide proactive intervention and coordination of care to eligible Vaya Health members. • Conduct assessments and identify service needs for members. • Coordinate care planning with multidisciplinary teams. • Ensure compliance with HIPAA requirements and other regulatory guidelines. • Monitor service delivery and member satisfaction. • Provide transition planning assistance and follow-up care coordination.
• Bachelor’s degree in health, psychology, sociology, social work, nursing, or relevant human services area preferred. • Minimum two years of experience serving members with behavioral health (BH) conditions. • Minimum two years of experience serving members with intellectual/developmental disabilities (I/DD) or traumatic brain injury (TBI). • Minimum two years of prior Long-term Services and Supports and/or Home Community Based Services coordination, care delivery monitoring, and care management experience. • Must meet criteria of being a North Carolina Qualified Professional with the population served in 10A NCAC 27G .0104. • Experience with NC Medicaid program, NC Medicaid Transformation, and Tailored Plans preferred.
