Beschreibung

• Develop proactive plans of care to address identified issues and improve short- and long-term outcomes and overall wellness • Evaluate member needs and benefits using clinical tools and information/data review • Apply clinical judgment to reduce risk factors and barriers and address complex health and social indicators • Conduct holistic assessments using claims and other sources, including co-morbid and multiple diagnoses affecting functionality • Assess need for referrals to clinical resources and interdisciplinary team members • Collaborate with supervisors, providers, community organizations, and other stakeholders to overcome barriers and meet goals • Present cases at interdisciplinary case conferences • Apply case management processes in compliance with regulatory and company policies and procedures • Use motivational interviewing to maximize member engagement and assess health status and needs • Interact with members and clients by telephone or in person • Provide ongoing case management services in members' homes, worksites, or physician offices as needed • Travel within a designated geographic area for in-person case management activities based on leadership direction and business needs

🎯 Anforderungen

• Active Registered Nurse license in good standing in Illinois • Minimum 3-5 years of clinical practical experience • Minimum 2-3 years of case management, discharge planning, and/or home health care coordination experience preferred • Excellent analytical and problem-solving skills • Effective communication, organizational, and interpersonal skills • Ability to work independently from home • Proficiency with MS Word, Excel, Outlook, PowerPoint, and proprietary applications • Efficient computer skills, including navigating multiple systems and keyboarding • Reliable transportation • Willingness and ability to travel up to 40% of the time • Associate degree required; bachelor's degree preferred • Direct/hardwired internet connection to a modem/router within 7 feet of the computer, with minimum speeds of 25 Mbps download and 3 Mbps upload • WiFi and satellite internet are not permitted • Quiet, secure, private, designated home workspace compliant with CVS Health and HIPAA guidelines • Must work within the state and city where currently living • Must provide own workspace furniture • Ability to set up and use multiple monitors and navigate multiple applications • Ability to communicate through email, calendar invites, Teams messaging, and virtual meetings • Experience with Microsoft Office 365 applications or similar Google Workspace applications • Ability to log in to secure systems such as VPN and EHR portals, secure devices, manage strong passwords, and recognize suspicious emails or links • Ability to resolve common technical issues independently and contact IT for unresolved issues • Openness to learning new workplace skills • Certified Case Manager and additional national professional certification (CRC, CDMS, CRRN, COHN, or CCM) preferred
