📋 Description

• Manage individualized patient care through the nursing process • Collaborate with physicians and multidisciplinary team members • Provide physical and psychological support to patients and families • Assess, treat, manage, and educate patients with acute or chronic health needs in the home setting • Safely and efficiently deliver infusion therapy and administer biologic medications • Analyze patient data and lab work; report changes to referring physicians and recommend patient care • Perform initial and ongoing patient assessments and provide patient education • Facilitate communication among healthcare team members • Incorporate patients and families into care planning and treatment • Administer medications, infusion therapy, and other treatments per prescriber orders • Obtain referrals, schedule home health visits, and confirm appointments in the EMR • Document and verify physician orders, assessments, responses to interventions, and progress toward infusion therapy goals • Participate in quality initiatives and program reviews

🎯 Requirements

• Bachelor’s or Associate degree in Nursing • Current, active and unrestricted RN license in the state of practice • Current CPR certification (BLS minimum) • Minimum of one year experience providing nursing care • Infusion experience • Telephone, MS Office and computer/keyboard skills • Knowledge of operations processes and guidelines for communication and PHI • Ability to conduct accurate assessments, prioritize, manage time, think critically, make decisions, solve problems, and communicate clearly and accurately • Previous EMR experience preferred
