📋 Description

• Provide care coordination for Medicaid, Medicare Advantage, and/or Dual Eligible Special Needs Plan (DSNP) members • Build trusted relationships and create a positive member experience • Help members access appropriate care through health plans, providers, pharmacies, vendors, and community-based organizations • Assess members' medical and social needs, including Social Determinants of Health • Conduct outreach to motivate, facilitate, and educate members about program benefits • Conduct telephone and/or text assessments and develop care plans with members • Evaluate individual care needs and communicate medical information to healthcare professionals • Manage a caseload and ensure timely member contact • Coordinate care with providers and schedule appointments as needed • Motivate members to engage in their health and overall well-being • Coordinate and complete correspondence according to established workflows • Document actions accurately in a care management platform • Conduct high-volume outreach to members, families, providers, and other recipients • Advocate for, support, and motivate members to manage their health and healthcare • Maintain relationships with members, families, providers, and external partners

🎯 Requirements

• Active and unrestricted Licensed Vocational Nurse / Licensed Practical Nurse (LVN/LPN) or Registered Nurse (RN) license • 2+ years of experience as a care coordinator, case manager, community health worker, pharmacy technician, or social worker • Must be located in Texas, Louisiana, Mississippi, Arkansas, Missouri, Colorado, Oklahoma, Kansas, Utah, Nebraska, or Wyoming • Dedicated home office for remote work • 1+ years of remote work experience • Experience working with Medicaid and/or Dual Eligible patients • Strong interpersonal, verbal and written communication, and problem-solving skills • Experience working with customers by phone and text message • Experience documenting case notes in a care management or electronic health record platform • Experience with motivational interviewing • Experience with Apple computers, such as MacBook, preferred • Bilingual fluency in Spanish preferred • Knowledge or experience in post-discharge follow-up preferred • Knowledge or experience in chronic disease management preferred • Ability to work effectively in a multicultural setting with diverse populations • Ability to advocate for, support, and motivate members • Ability to maintain positive working relationships with members, families, providers, and external partners • Ability to build trust, actively listen, communicate intentionally, collaborate, and problem-solve • Ability to influence others toward positive outcomes • Passion for helping people improve their lives
