• Provide professional and friendly proactive care and triage for clinical issues • Embed a family-centered care philosophy in care delivery • Demonstrate cultural competence and sensitivity when working with culturally diverse populations • Coordinate transitions of care for ED/IP/UC care with clinical providers following discharge • Perform comprehensive assessments of clinical, psychosocial, discharge planning, and financial needs • Establish rapport with patients and families, understand their needs and expectations, and set realistic mutual goals • Establish comprehensive plans of care with physicians, patients, and the interdisciplinary team • Communicate plans of care and related changes to patients, families, physicians, and healthcare team members • Gather information to evaluate plan-of-care effectiveness and ensure safe, timely, accurate, and cost-effective care • Document care management assessments and interventions • Refer complex psychosocial and discharge planning issues to Social Work or Behavioral Health and coordinate appropriate follow-up • Consult interdisciplinary team members, including dietary and pharmacy, to support safe discharge • Perform other duties as assigned
• Licensed RN in at least one state, with eligibility to register for other state licensures • Bachelor's in nursing from an accredited university required • 5+ years of pediatrics experience required • 1+ years of care coordination or case management experience preferred • Bilingual Spanish required • Familiarity with Medicaid regulations and services a plus • Value Based Care (VBC) experience a plus • Virtual care experience a plus • Must physically work in the United States • Ability to work the expected 3x12 schedule: Monday-Wednesday or Wednesday-Friday, 7:00am-7:00pm CST
