📋 Description

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members • Develop actionable, individualized, and measurable care plans based on member needs and identified health risks • Establish health coaching relationships and provide education, encouragement, and support related to chronic condition management • Use clinical nursing judgment to assess member needs and coordinate interventions for acute situations • Support transitions of care and safe discharge planning, including coordination of DME, home health services, provider appointments, and other resources • Help members and caregivers navigate healthcare benefits, services, and care options • Assess physical, environmental, and psychosocial factors affecting members’ health and well-being • Collaborate with multidisciplinary teams, including social workers, dietitians, pharmacists, providers, and other healthcare professionals • Promote member self-management through education and individualized resources • Coordinate care and condition-management activities among members, caregivers, providers, and care team members • Document member interactions, assessments, care plans, interventions, and outcomes accurately and timely in applicable systems

🎯 Requirements

• Active, unrestricted Registered Nurse (RN) license in the state of residence • Active Compact RN license permitting practice in applicable compact states • Minimum of 2 years of clinical nursing experience; 3 or more years preferred • Strong clinical judgment, critical-thinking, and problem-solving skills • Ability to work independently and exercise appropriate nursing judgment in a remote environment • Intermediate to advanced computer proficiency, including Microsoft Word, Excel, and Outlook • Ability to efficiently navigate multiple computer systems and electronic medical records • Strong verbal and written communication skills • Experience caring for elderly and/or chronically ill populations preferred • Experience in care coordination, case management, or managed care preferred • Experience with Medicare and/or Medicaid populations preferred • Experience in chronic condition management and patient education preferred • Experience in telephonic nursing, health coaching, or remote care management preferred • Experience working as part of a multidisciplinary healthcare team preferred
