📋 Description

• Conduct home and virtual visits, including telehealth and telephonic visits • Develop wellness coaching relationships with patients, caregivers, and families • Contact patients eligible for Alignment’s Care at Home program and provide wellness coaching to reduce or eliminate high-risk behaviors • Collaborate with advanced practice clinicians and/or social workers to develop action plans • Encourage habits conducive to a higher quality of life • Deliver a high level of patient service and increase member retention and satisfaction • Assist patients with accessing community resources and social services addressing social determinants of health and wellbeing • Develop and distribute health education materials to patients, caregivers, and families • Coordinate care with Alignment team members and health-related vendors for optimal patient care

🎯 Requirements

• Minimum 1+ year of experience as a wellness or health coach or educator • Senior care experience preferred • High School Diploma or GED required; Associate’s degree or two years additional experience in lieu of education • Bachelor’s Degree preferred • Ability to communicate positively, professionally and effectively with others; provide leadership, teach and collaborate with others • Solid knowledge of specific diseases, lifestyle-related topics, nutrition, and exercise • Knowledge of and experience with applying the stages of change or a similar behavior change model • Excellent interpersonal skills with the ability to foster trust and respect among customers and coworkers • Able to work independently and remotely • Able to work with protected health information and maintain absolute confidentiality and privacy in accordance with HIPAA regulations and AHP policies • Proficient computer skills including Microsoft Office Excel, Word and PowerPoint • Ability to work a flexible schedule • Effective written and oral communication skills; ability to establish and maintain a constructive relationship with diverse members, management, employees and vendors • Ability to perform mathematical calculations and calculate simple statistics correctly • Ability to prioritize multiple tasks; advanced problem-solving; ability to use advanced reasoning to define problems, collect data, establish facts, draw valid conclusions, and design, implement and manage appropriate resolution • Effective problem solving, organizational and time management skills and ability to work in a fast-paced environment • Bilingual skills preferred • No licensure required • Regularly required to talk or hear; stand, walk, sit, use hands, handle or feel objects, and reach with hands and arms • Frequently lifts and/or moves up to 10 pounds • Close vision and ability to adjust focus required
