📋 Description

• Assist the Clinical Pharmacy Team with pharmacy utilization and implementation of medication management programs • Maintain and improve the Pharmacy Quality program • Improve patient outcomes and promote safe and effective medication use under pharmacist supervision • Support CMS/HEDIS Quality STAR measures by targeting patients with chronic conditions • Support evidence-based pharmaceutical care • Conduct outbound and receive inbound member and provider calls regarding gaps in care, adherence, and therapeutic interchange • Serve as a contact and subject matter expert for clinical pharmacy programs and pharmacy benefit management issues • Document clinical activities and outcomes in the electronic health record (EHR) • Communicate with clinical staff, provider offices, health plan partners, and in-network pharmacies • Assist with data reviews, reports, and presentations • Support ad-hoc projects and perform other assigned duties • Research, process, and resolve drug authorization requests and respond to physician inquiries

🎯 Requirements

• National Pharmacy Technician Certification (PTCB or NHA) • HS Diploma • 2-3 years of Pharmacy related experience • Preferred: Experience with CMS/HEDIS Quality STAR measures • External hires must pass a background check/drug screen • Must be able to work M-F / 8AM - 5PM PST
