📋 Description

• Provide personalized, coordinated health care to long-term care residents enrolled in special Medicare Advantage plans • Exercise independent judgment to treat patients with acute and chronic conditions • Consult with a supervising physician as appropriate • Collaborate with providers, RN case managers, facility partners, and health plan leadership • Implement the national model of care in compliance with federal and state regulations • Assess patients’ medical and mental health needs • Develop plans of care that proactively manage conditions and address barriers to care • Ensure compliance with local, state, and federal agencies • Prescribe medications and order lab work, diagnostic procedures, and consultations • Monitor treatment compliance and response, modifying treatment plans as needed • Provide training and advice to facility staff when required

🎯 Requirements

• FNP, AGNP, AHACNP or PA license required • Degree from an accredited APRN or PA program • 3 years’ experience in clinical nursing or rehab in geriatric populations • Electronic Health Records experience • Working knowledge of Microsoft applications, including Word, Outlook and Excel • Experience in primary care or internal medicine preferred, preferably with geriatrics focus • Masters or better • APRN Advanced Practice RN certification/license required • Experience in Primary Care in Geriatric Medicine preferred • Experience in Long Term Care preferred
