📋 Description

• Guide patients through the healthcare system, including scheduling appointments, referrals, and follow-up care coordination • Serve as a primary point of contact for patients, addressing questions and concerns with empathy and professionalism • Assist patients in understanding treatment plans, discharge instructions, and available healthcare resources • Collaborate with clinical staff, physicians, and administrative teams to ensure continuity of care • Maintain accurate and up-to-date patient records in the electronic health record (EHR) system • Identify and help remove barriers to care, including insurance, transportation, and social determinants of health • Monitor patient progress and proactively follow up to ensure adherence to care plans

🎯 Requirements

• High school diploma or GED required • Associate's or bachelor's degree in healthcare, social work, or related field preferred • Prior experience in a healthcare, patient services, or care coordination setting strongly preferred • Familiarity with medical terminology, EHR systems, and healthcare workflows • Strong interpersonal and communication skills with a patient-first mindset • Ability to multitask, prioritize, and problem-solve in a dynamic environment • Knowledge of community health resources, insurance processes, and care management principles • Bilingual candidates are a plus
