📋 Description

• Assess, plan, implement, coordinate, monitor, and evaluate case management options and services for members’ health needs • Coordinate with departments, providers, and community-based organizations to promote effective and efficient care • Serve as a liaison to facilitate quality of care, cost effectiveness, accessibility, and appropriate service levels • Practice within licensure scope and comply with policies, procedures, regulations, URAC standards, and state regulations • Maintain current knowledge of social work licensing requirements and continuing education • Serve as a resource for non-clinical staff • Travel to sites or conduct home visits when required • Proactively process referrals from multiple sources, identify case-management candidates, explain services, obtain voluntary agreement, and maintain a timely patient caseload • Perform other duties as assigned

🎯 Requirements

• Master’s degree in Social Work • Current active Licensed Master Social Worker (LMSW) license in good standing in the state(s) where job duties are performed • Case Management Certification required; if not obtained before employment, successful testing required within three years • Valid driver’s license • Arkansas Licensed Clinical Social Worker (LCSW) preferred • Minimum two years’ clinical practice experience in direct clinical care with patients in a hospital, facility, or community-based environment • Minimum two years’ experience in case management, home health, critical care, social work, discharge planning, or utilization review • Proficiency with Microsoft Office, including Word, Excel, and Outlook • Ability to navigate multiple systems and perform keyboarding • Ability to make sound judgments and decisions based on facts and guidelines • Ability to build collaborative relationships • Attention to detail • Ability to interpret complex documentation • Ability to work independently with little supervision • Ability to maintain security and confidentiality of records and information • Ability to adhere to segregation-of-duties guidelines • Ability to routinely travel for work and meet periodic lifting and carrying requirements up to 40 lbs
