📋 Description

• Verify insurance eligibility • Conduct outreach calls to members to coordinate care after inpatient hospitalization or emergency room discharge • Coordinate referrals to Care Management following inpatient hospitalization or emergency room discharges • Work with patients, providers, and insurance carriers to ensure timely access to services and continuity of care • Research concerns and provide resolutions to patient issues and needs • Collect and document client data in the medical record • Engage multidisciplinary team resources to achieve optimal patient, family, and caregiver outcomes • Coordinate multiple appointments using scheduling systems • Provide care-gap lists to the multidisciplinary team and coordinate closure of gaps/issues • Follow up with members regarding needs and obstacles, health literacy, cultural issues, and social-class barriers • Meet and accompany patients and families to initial provider appointments before and after acute and post-acute transition • Establish collaborative relationships with physicians, payers, and healthcare team members • Collect and communicate timely information to payers to fulfill utilization and regulatory requirements • Educate internal healthcare team members on care management and managed care concepts • Facilitate integration of concepts into daily practice • Work independently to provide excellent patient care

🎯 Requirements

• High school diploma/GED or equivalent working knowledge • Medical terminology knowledge • Completion of Medical Assistant Training, Patient Care Technician Training, or Certified Nursing Assistant Training • 2 years of health care experience • Excellent time management and problem-solving skills • Ability to meet constantly shifting deadlines and timelines • Excellent customer service and recovery skills • Bachelor’s degree in Social Work preferred • Additional related education and/or experience preferred • Candidates must reside in Arizona
