📋 Description

• Partner with members and families to identify supports and direct services for health, employment, and independent or community living goals • Collaborate with clinical and social support colleagues to address members’ physical, behavioral health, and long-term service needs • Conduct thorough, objective face-to-face home and community visits • Assess members’ physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs • Use criteria to authorize appropriate home- and community-based services • Confirm authorized services are provided and members’ needs are met • Travel to members’ homes for visits • Manage requests and demands from internal and external stakeholders • Participate in the BCBST Driver Safety Program when routinely operating a company, personal, or rental vehicle for company business • Maintain compliance with applicable driver safety, insurance, and motor vehicle record requirements

🎯 Requirements

• 3 years of experience in a clinical setting • 2 years of clinical experience required • Registered nurse with an active license in Tennessee, or an active license in the employee’s state of residence if participating in the Nurse Licensure Compact Act • Alternatively, Master’s in Social Work with an active unrestricted LCSW, LMSW, or LAPSW license • Exceptional customer service skills • Must live in Rhea County, Tennessee • Available for an 8:00 a.m.–5:00 p.m. EST schedule • Valid driver’s license • TB Skin Test applies to coordinators working in the field • Basic PC skills, Microsoft Office, and email proficiency • Effective time management skills • Excellent oral and written communication skills • Strong interpersonal and organizational skills • Digital literacy assessment required • Various immunizations and/or associated medical tests may be required
