📋 Description

• Lead and supervise a team of TOC Coordinators supporting transitions of care • Advocate for patients transitioning home from hospital and/or post-acute settings • Help prevent patient readmissions • Dedicate approximately 30% of time to providing TOC patient care • Connect with patients and enable ancillary services, including appointments, transportation, supplies, and durable medical equipment • Support day-to-day team operations and participate in rounds/huddles • Develop workflows and job aids • Coach and manage team performance • Monitor attendance, adherence, meeting participation, and relevant Slack channels • Collaborate with TOC leadership to develop the program • Represent TOC coordination in cross-functional meetings • Assist with recruiting and hiring new TOC Coordinators • Participate in and coordinate clinical model pilots • Partner with trainers and DM education team to onboard new TOC Coordinators • Conduct 1:1s and answer coordinator questions • Assist with process improvement, workflow implementation, job aid creation, and training • Meet regularly with the Senior Manager to review team performance and recommend improvements

🎯 Requirements

• MA Certification (desired); expired MA certifications are OK • A copy of your active or expired certification will be required for verification • If no certification, significant MA experience/background may be accepted; MA school diploma and transcripts may be requested • 2 years of direct lead or supervisory experience in a virtual setting required • 2+ years healthcare administrative/office experience • Proficient in technology using evolving platforms • Organized with high attention to detail • Fully bilingual (English/Spanish) preferred but not required
