📋 Description

• Lead and manage a team of Transitions of Care Coordinators • Provide TOC patient care for approximately 30% of the time • Support patients transitioning home from hospital or post-acute settings and help prevent readmissions • Connect with patients and enable ancillary services including appointments, transportation, supplies, and durable medical equipment • Monitor team attendance, adherence, and participation in meetings/forums • Collaborate with TOC leadership to develop the program • Represent TOC coordination in cross-functional meetings and share relevant updates • Assist in recruiting and hiring new TOC Coordinators • Participate in and coordinate TOC clinical model pilots • Partner with the TOC trainer and DM education team to onboard new coordinators • Manage coordinator performance across key performance indicators • Answer questions, monitor relevant Slack channels, and conduct 1:1s • Design and implement workflows, create job aids, and facilitate training • Meet regularly with the Senior Manager to review team performance and recommend improvements • Attend or facilitate meetings with patients and leadership as required

🎯 Requirements

• MA Certification desired; expired MA certifications are OK, with verification of a copy of active or expired certification required • If no certification, significant MA experience/background may be considered, with possible submission of MA school diploma and transcripts • 2 years of direct lead or supervisory experience in a virtual setting required • 2+ years healthcare administrative/office experience • Proficient in technology using evolving platforms • Organized with high attention to detail • Fully bilingual (English/Spanish) preferred but not required
