📋 Description

• Support members’ physical, behavioral, and social health and well-being • Assist members in finding and maintaining competitive, integrated employment • Navigate challenging relationships to ensure members receive necessary services • Perform intake, enrollment, and ongoing assessments • Develop and maintain Person-Centered Plans • Monitor and maintain overall service provision • Partner with members and families to identify supports and direct services for health, employment, and independent or community living goals • Collaborate with clinical and social support colleagues to meet members’ physical, behavioral health, and long-term service needs • Conduct thorough, objective face-to-face visits and assess members’ physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs • Apply criteria to authorize appropriate home- and community-based services • Confirm authorized services are provided and members’ needs are met • Work an 8:00 a.m.–5:00 p.m. schedule with no on-call responsibility • Travel to members’ homes in the Greene, Hamblen, and Jefferson County area

🎯 Requirements

• RN with preference for current Developmental Disabilities Nurses Association certification as a Certified Developmental Disabilities Nurse (CDDN) for RNs or Developmental Disabilities Nurse (DDC) for LPNs, as applicable • Or B.S. in social work, nursing, education, human service, health care profession, or another related field • Or meet the federal requirements for a Qualified Developmental (or Intellectual) Disabilities Professional (QDDP or QIDP) • Or, in lieu of a degree, 5+ years’ experience as an independent Support Coordinator or case manager for service recipients in a 1915(c) HCBS Waiver and completion of Personal Outcome Measures Introduction and Assessment Workshop trainings established by the Council on Quality and Leadership • Licensed Master Social Worker or Licensed Clinical Social Worker preferred • 2 years of experience in the healthcare industry or an operations-type environment required • Valid driver's license • Various immunizations and/or associated medical tests may be required • Ability to work independently with minimal supervision or in a team environment • Proficiency in Microsoft Office (Outlook, Word, Excel, and PowerPoint) • Ability to solve problems and manage multiple assignments with critical deadlines • Organized, reliable, and able to manage multiple tasks • Excellent oral and written communication skills • Strong interpersonal and organizational skills • Digital literacy assessment required • Employees who routinely operate a company, personal, or rental vehicle must maintain a valid driver's license, comply with auto insurance requirements, and maintain an acceptable motor vehicle record
