📋 Description

• Provide crisis intervention and help families understand the complexities and impact of medical situations • Assess patients' and families' ongoing needs for and responses to healthcare services • Interview patients and plan activities addressing their social and emotional needs and facilitating psychological, emotional, and physical recovery • Conduct telephonic outreach, care coordination, advocacy, and connect members with community and healthcare resources • Support elderly and disabled members with complex medical, behavioral, and social needs • Partner with members, providers, caregivers, and community organizations to address barriers to care and improve health outcomes • Coordinate with external agencies and home care nursing groups for post-discharge care • Perform home visits when necessary

🎯 Requirements

• Must hold an active Tennessee Social Work license • Clinical Licensure or Education Master's degree required • One of the following unrestricted TN licensures: LCSW or LMSW • Valid Driver's License required • 3 years experience providing care coordination to persons receiving long-term care and/or home and community based services required • Various immunizations and/or associated medical tests may be required • Experience in skilled nursing facilities (SNF), acute inpatient settings, discharge planning, community resource coordination, or hospice care is particularly of interest • Must be detail oriented, deal well with ambiguity, and demonstrate strong strategic thinking and creativity skills • Effective communication with internal and external partners/customers • Awareness of the impact of cultural/socioeconomic status pertaining to healthcare and mental healthcare services • Must be able to perform home visits if necessary • Employees who routinely operate a company, personal, or rental vehicle must participate in the BCBST Driver Safety Program, maintain a valid driver's license and compliant auto insurance, and have an acceptable motor vehicle record • CCM certification is preferred but not required
