📋 Description

• Provide routine telehealth visits with Medicare patients, including E/M and Chronic Care Management • Create and manage personalized care plans for ongoing conditions • Collaborate with the integrated care team, including nurse case managers, NPs, and CHWs • Ensure thorough documentation in the EHR and compliance with quality measures • Engage patients in preventive care to reduce avoidable complications

🎯 Requirements

• MD or DO • Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics • Active medical license in Georgia • Multi-state licensure a plus • Credentialed with major plans: UHC, Aetna, Humana • Strong interest in preventive care, senior health, or chronic condition management • Tech-savvy and comfortable practicing via telehealth • Collaborative, mission-driven mindset with excellent communication skills
