📋 Description

• Screen, assess, and manage patients for behavioral health symptoms and conditions in the Advocacy Care Model • Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, and motivational interviewing • Manage clinical escalations and complex cases within the team • Participate in caseload consultation with the psychiatric consultant • Communicate treatment recommendations to the patient’s medical provider • Serve as an ongoing liaison between providers to coordinate patient care • Track treatment response, clinical symptoms, and treatment side effects • Coordinate care with referral sources as clinically indicated • Support primary care practices and partners with comprehensive patient population needs • Oversee a patient panel and provide support to unlicensed care managers

🎯 Requirements

• Master’s in Social Work or Mental Health Counseling required • LICSW or LMHC license in state of Massachusetts required • Minimum of 3-5 years of experience of direct clinical practice in behavioral health required • Previous experience providing measurement-based care preferred • Previous experience managing clinical escalations and performing safety risk assessments preferred • Previous experience working with patients with serious mental illness and substance use disorder preferred • Comfortability and experience working with children and adolescents preferred • Previous experience with AIMS Collaborative Care Model preferred • Ability to build trust and rapport with patients and stakeholders • Ability to manage multiple patient cases and required documentation concurrently • Willingness to adjust to changing priorities and demands • Ability to use and document in electronic health systems and utilize desktop applications for communication
