• Develop and maintain protocols for prescribing, dispensing, and administering medications • Conduct research to develop or improve pharmaceutical treatments • Provide specialized services for patients managing Diabetes, Chronic Obstructive Pulmonary Disease, Congestive Heart Failure, Asthma, or Hypertension • Participate in quality improvement initiatives • Develop or revise pharmacy policies and procedures • Provide training and support to pharmacy staff and interns • Maintain current knowledge of new drugs, medical devices, and therapies • Ensure compliance with federal, state, and local pharmaceutical regulations • Consult with physicians on drug prescribing practices • Participate in patient intervention and referral, risk management, and ethical decision-making • Participate in multidisciplinary team meetings and contribute to patient care plans • Collaborate with healthcare professionals to plan, monitor, review, and evaluate patient effectiveness • Educate patients about medications, side effects, interactions, and therapeutic effects • Review patient profiles to ensure medication safety and efficacy • Participate in patient care rounds and perform drug therapy consultations • Report to the Manager of Pharmacy Services • Manage professional development opportunities within the pharmacy department • Perform additional duties as assigned
• A doctor of pharmacy degree from an accredited institution • Licensure as a pharmacist in the state of practice • 3 years of experience in a pharmacy setting • Proven experience as a clinical pharmacist • Excellent understanding of pharmaceutical therapy and direct patient care • Ability to manage multiple priorities in an expedient and decisive manner • Proficient with Google Suite (Drive, Docs, Sheets, Slides) and Microsoft Office (Word, Excel, PowerPoint) for real-time collaboration • Requires standing, walking, pushing, bending, kneeling, and reaching • Ability to sit for extended periods of time • Requires corrected vision and hearing to normal range • Ability to operate a motor vehicle and have own means of transportation • Demonstrated ability to handle data with confidentiality • Ability to work cross-functionally with multiple teams and independently with minimal supervision • Excellent organizational, time-management, and multi-tasking skills with strong attention to detail • Excellent written and verbal communication skills; comfortable communicating with senior-level leadership, providers, and health plans • Strong interpersonal and presentation skills • Strong critical thinking and problem-solving skills • Results-oriented with a focus on quality execution and delivery • Appreciation of cultural diversity and sensitivity toward target patient populations
