📋 Description

• Provide care to patients discharged to post-acute facilities • Maintain a caseload according to productivity standards • Coordinate with patients, families or support members, inpatient case management, and interdisciplinary care team members • Support appropriate utilization of services, length of stay, and safe discharge planning • Run daily transitions-of-care reports, make assignments, and review goals of care • Communicate at least weekly with skilled nursing facilities • Evaluate care plan effectiveness with the Interdisciplinary Team • Conduct comprehensive assessments and complete required documentation • Perform patient outreach and document in the electronic medical record • Coordinate referrals and transitions between providers and care settings • Provide patient and family or caregiver education • Refer patients to the appropriate next level of care and support discharge planning • Conduct monthly performance, quality, and readmission meetings with high-volume facilities • Assist with standard operating procedures and policy development

🎯 Requirements

• Associates of Science, Nursing (ADN) (required) • Current RN License in the state in which they are working or covered by compact (required) • Nursing compact/multistate license prior to starting • 2 years of nursing experience (required) • 1 year of experience in skilled nursing settings (preferred) • Bachelors of Science, Nursing (BSN) (preferred) • Certification for care management (preferred)
