📋 Description

• Lead planning, implementation, operationalization, scaling, and continuous improvement of enterprise-wide program management initiatives • Lead development and implementation of enterprise programs aligned with strategic priorities and value-based care objectives • Develop program charters, roadmaps, implementation plans, business cases, budgets, operational models, policies, procedures, specifications, requirements, schedules, and status reports • Coordinate multidisciplinary teams across clinical operations, population health, specialty care, ambulatory practices, virtual care, digital health, analytics, and information technology • Engage stakeholders and lead change management to drive adoption of new workflows and care models • Establish governance structures, milestones, risk mitigation plans, and executive reporting processes • Assess operations and develop implementation strategies for program readiness, compliance, and performance • Identify process improvement opportunities and recommend corrective actions • Lead product and service go-live events and analyze market relationships and needs • Optimize and coordinate system-wide initiatives with vendors, informatics, and frontline service providers • Implement and manage standardized processes, reporting, and approaches across organizational divisions • Track and monitor KPIs and business-to-business operations data; interpret and report results to management at regional, market, and system levels • Support quality control standards and policy work, including reporting, process and control evaluation, remediation, and monitoring • Facilitate and lead teams, groups, meetings, and discussions • Maintain knowledge of healthcare trends, compliance regulations, standards, and directives • Provide feedback to subject matter experts on products and services and suggest future program development • Maintain personal and professional development

🎯 Requirements

• 4 year/Bachelor's degree in healthcare, business administration, finance, analytics, informatics, or related field (required) • 5+ years of experience in healthcare program management, clinical operations, population health, quality improvement, or project management (required) • Knowledge of chronic disease management • Knowledge of value-based care models • Knowledge of healthcare operations and delivery • Experience with EMR systems, preferably EPIC, and integration • Program and project management skills • Ability to develop KPIs, dashboards, and performance improvement plans • Ability to lead through influence and lead multidisciplinary teams • Executive-level stakeholder engagement • Change management • Ability to scale operational initiatives across multiple sites of care • Lean principles • Verbal, written, and presentation communication skills • Strong analytical, organizational, and problem-solving skills • Must work day shift / United States of America work shift
