📋 Description

• Assess patient needs telephonically using nursing processes and established protocols • Determine the appropriate level of care and provide patient education • Establish plans of care, interventions, and follow-up instructions • Escalate and collaborate with on-call providers when additional guidance is needed • Prioritize patient interactions by acuity and need • Apply evidence-based practice to deliver patient care • Implement strategies to reduce patient risk and increase patient safety • Assess patient and family readiness to learn and individualize education • Support shared governance and process-improvement initiatives • Participate in professional development, meetings, educational programs, and annual competencies • Communicate effectively and promote collaboration with clinicians and healthcare team members • Identify and report patient safety events and near misses • Apply ethical decision-making and participate as an effective member of the patient care team • Provide age-appropriate care advice and disposition according to department policies, procedures, and protocols • Schedule appointments according to protocol recommendations • Coordinate medical and nursing management, including procedures and medication refills • Maintain and update clinical records according to agency, state, and federal guidelines • Document call encounters in the Electronic Medical Record at the time of the call • Communicate patient status and care-plan responses to physicians, advanced practice clinicians, and interdisciplinary team members • Participate in team meetings and special projects assigned by leadership • Contribute to health information resources, system efficiencies, and operational improvements • Assist with interpreting department policies and advising staff on procedural changes

🎯 Requirements

• Registered Nurse license issued by the state in which the team member practices • Associate's Degree (or equivalent knowledge) in Nursing • 2+ years of acute care experience, preferably within the last 4 years; ED, Urgent Care, ICU and some Med Surg will be considered • Acute care experience within the last 4 years is required for consideration • Critical thinking and problem-solving skills • Knowledge of healthcare specialties, levels of care, symptom identification, treatment recommendations, and established protocols • Excellent verbal communication skills, including empathy, active listening, diplomacy, and open-ended questioning • Ability to develop rapport and maintain professional relationships with patients, staff, and physicians • Ability to organize and prioritize work and manage multiple priorities • Customer service and follow-up skills, including remaining calm during stressful situations • Proficiency with computers, internet, desktop software packages, and multiple-line telephone systems • Ability to research and document calls on multiple systems • Knowledge of documentation techniques for communication • Stable and secure internet connection • Functional vision, touch, speech, and hearing • Quiet space to make and receive phone calls • Ability to lift 15 lbs • Availability to work every 4th weekend and holiday rotation • Availability for day start times of 8a–10a or PM start times of 12p–2p or later CST
