• Use the nursing process and established protocols to assess patient needs telephonically. • Determine the appropriate level of care, provide education, establish care plans and interventions, and communicate follow-up instructions. • Escalate cases and collaborate with on-call providers when additional guidance is needed. • Prioritize patient interactions by acuity and need. • Apply evidence-based practice and strategies that reduce risk and improve patient safety. • Assess patient and family readiness to learn and individualize education approaches. • Support shared governance, quality improvement, process improvement, and patient outcome initiatives. • Schedule appointments according to protocol recommendations. • Coordinate medical and nursing management, procedures, and medication refills with healthcare team members. • Maintain and update clinical records and document all call encounters in the Electronic Medical Record. • Communicate patient status and concise care-plan reports to physicians, Advanced Practice Clinicians, and interdisciplinary team members. • Participate in team meetings and special projects assigned by leadership. • Contribute to health information resources, system efficiencies, and operational improvements. • Assist with interpreting department policies and procedures and advise staff on procedural changes.
• Registered Nurse license issued by the state in which the team member practices • Associate's Degree (or equivalent knowledge) in Nursing • 2+ years of acute care experience, preferably within the last 4 years; ED, Urgent Care, ICU, and some Med/Surg considered • Critical thinking and strong problem-solving skills • Knowledge of healthcare specialties, levels of care, symptom identification, and treatment recommendations • Ability to apply established protocols and prior clinical experience • Excellent verbal communication skills, empathy, active listening, diplomacy, and customer service • Ability to organize and prioritize multiple tasks in a fast-paced environment • Proficiency with computers, internet, desktop software, multiple-line telephone systems, and multiple systems for research and documentation • Knowledge of documentation techniques • Stable and secure internet connection • Functional vision, touch, speech, and hearing • Ability to sit for most of the workday • Quiet space for making and receiving phone calls • Ability to lift 15 lbs. • Must be available to work every 4th weekend and holiday rotation • Must work an 8-hour or 10-hour shift, with start times of 8–10 a.m. or 12–2 p.m. CST or later
