• Support care management activities and assigned member teams • Ensure services are delivered by healthcare providers and partners • Perform member outreach by telephone or home visits • Discuss care plans/service plans, next steps, resources, questions, concerns, and ongoing education • Coordinate care activities with healthcare and community providers, partners, members, and caregivers • Support member and provider inquiries, requests, and concerns related to care plans/service plans • Communicate with care managers, practitioners, and others to facilitate member services and continuity of care • Support service assessments/screenings and document member care needs • Document and maintain member records according to state and regulatory requirements • Distribute records to providers as needed • Identify needs and make referrals to care managers, community-based organizations, and disease managers • Provide education on available benefits and resources • Perform other assigned duties and comply with all policies and standards
• Must reside in Kansas • Behavioral Health experience preferred • High School diploma or GED • 1–2 years of related experience • For Arkansas Total Care plan: Bachelor's degree in social science/health-related field OR high school diploma with at least one (1) year of experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients • For Arkansas Total Care plan: driver's license required • For Arkansas Total Care plan: child and adult maltreatment check required before hire and recurring • For Arkansas Total Care plan: drug screen required at time of hire and recurring • For Arkansas Total Care plan: must reside in Arkansas or a border city • For Arkansas Total Care plan: 30% travel required • Compliance with policies, contractual requirements, and regulatory guidelines and standards • Ability to identify needs and make referrals to Care Manager, community-based organizations, and Disease Manager
