📋 Description

• Develop, assess, and facilitate complex care management activities for primarily mental and behavioral health needs • Provide high-quality, cost-effective healthcare outcomes through personalized care plans and education for members and families • Evaluate member needs by phone or in-home visits and recommend or facilitate care/service plans • Conduct telephonic, digital, home, and other site-visit outreach • Develop ongoing care plans for high-acuity members and identify providers, specialists, and community resources • Coordinate among members, families/caregivers, community resources, and care provider teams • Monitor care plans, member status, treatment side effects, complications, and clinical symptoms • Facilitate care coordination and collaborate with providers and specialists to ensure timely access to care • Collect, document, and maintain member information and care management activities for regulatory compliance • Educate members and families on procedures, provider instructions, treatment options, referrals, and healthcare benefits • Provide leadership feedback on opportunities to improve care quality and cost-effectiveness • Perform other duties as assigned and comply with all policies and standards

🎯 Requirements

• Qualified applicants must reside in the state of Florida • Active Florida licensure as listed in the posting • Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing • 2–4 years of related experience • Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN based on state contract requirements • Behavioral health experience required • One year of related professional pediatric care experience • Monday–Friday, 8am–5pm Eastern Time Zone availability • Medicaid experience with Foster Care or adoptions strongly preferred
