📋 Description

• Develop, assess, and facilitate complex care management activities for primarily mental and behavioral health needs • Provide personalized care plans and education for members and their families related to mental health and substance use disorder • Evaluate member needs via phone or in-home visits and recommend or facilitate care plans/service plans • Perform telephonic, digital, home, and/or other site-visit outreach to assess member needs and collaborate with resources • Develop ongoing care plans for members with high acuity • Identify providers, specialists, and community resources needed for care • Coordinate among members, families/caregivers, community resources, and care provider teams • Monitor care plans, member status, outcomes, treatment side effects, complications, and clinical symptoms • Facilitate care coordination and collaborate with providers or specialists to ensure timely access to care • Collect, document, and maintain member information and care management activities for regulatory compliance • Educate members and families on procedures, provider instructions, treatment options, referrals, and healthcare benefits • Provide leadership feedback on opportunities to improve care quality and cost-effective delivery • Perform other duties as assigned

🎯 Requirements

• Candidates must reside in Reno, McPherson, Rice, or Harvey County to be considered • Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing • 2–4 years of related experience • Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN based on state contract requirements • Behavioral health experience required for RN candidates • Experience working with foster care populations or within child welfare systems is preferred • Compliance with all policies and standards
