📋 Description

• Develop, assess, and coordinate holistic care management activities for members with significant mental and behavioral health needs • Evaluate member service needs and develop or contribute to care plans and service plans • Educate members, families, and caregivers on available services, benefits, preventive care, treatment options, referrals, medications, and healthcare provider instructions • Evaluate complex and high-risk members and recommend plans of care • Act as liaison and member advocate between members/families, physicians, and facilities/agencies • Support members with mental and behavioral health conditions, including depression, bipolar disorder, schizophrenia, borderline personality disorder, PTSD, substance use disorder, self-injurious behavior, and psychiatric inpatient admissions • Perform frequent home and other site visits, once a month or more, to assess needs and collaborate with resources • Provide subject matter expertise and operational support for behavioral health activities, including crisis calls, mental health first aid training, field safety, de-escalation practices, and medication monitoring • Educate on and coordinate medical, behavioral, and social community resources • Coordinate service authorizations for services such as meals, employment, housing, foster care, transportation, and activities of daily living • Coordinate referrals and identify providers, specialists, and community resources • Maintain accurate documentation in the electronic care management system • Support compliance with clinical, state, and federal guidelines • Provide leadership feedback to improve quality of care and cost-effective service delivery • Perform other assigned duties

🎯 Requirements

• Master's degree in Mental Health or Social Work, or graduation from an Accredited School of Nursing • 2–4 years of related experience • Licensed Behavioral Health Professional or RN based on state contract requirements, e.g., LCSW, LMFT, LMHC, LPC, or RN with behavioral health experience • For Sunflower plan: Licensed Behavioral Health Professional or RN based on state contract requirements, e.g., LMSW, LCSW, LMFT, LMHC, LPC, or RN with behavioral health experience • For Arkansas Total Care plan: driver's license, child and adult maltreatment check before hire and recurring, and drug screen at hire and recurring • For Arkansas Total Care plan: must reside in Arkansas or a border city • Ability to travel 5% for Arkansas Total Care plan • Compliance with all policies, standards, clinical guidelines, and current state and federal guidelines
