• Develop, assess, and coordinate holistic care management activities for quality, cost-effective healthcare outcomes • Develop or assist with personalized service care plans for long-term care members • Educate members and families/caregivers on available services, benefits, procedures, healthcare instructions, referrals, and service options • Evaluate member needs and available resources and recommend or facilitate appropriate plans • Identify providers, specialists, and community resources for long-term care • Coordinate between members/families/caregivers and care provider teams to ensure services are accessible • Provide resource support for employment, housing, participant direction, independent living, justice, and foster care needs • Monitor care plans, member status, and outcomes; recommend updates based on member needs • Interact with long-term care healthcare providers and partners • Collect, document, and maintain member information and care management activities for regulatory compliance • Perform home and/or other site visits to assess member needs and collaborate with providers and partners • Provide feedback to leadership on opportunities to improve quality of care and cost-effective service delivery • Perform other duties as assigned and comply with policies and standards
• Must reside in Lyon, Osage, and/or Coffey County, Kansas • Bachelor's degree and 2–4 years of related experience; or bachelor's degree in health, human, social work, or education services with one or more years of qualifying experience • Alternatively, high school degree or equivalent and three years of qualifying experience in case management of aged people, including behavioral health conditions, physical or developmental disabilities, or HIV/AIDS populations • Equivalent experience may be accepted through applicable knowledge, duties, scope, and skills • Field-based work • Technological savvy • Excellent communication and customer service skills • HCBS waiver knowledge • Strong time management skills • Team player • Knowledge of community-based resources and referrals • Problem-solving skills • Assessment and care-planning skills • Flexibility • Ability to comply with current state, federal, and third-party payer regulations, policies, and standards
