📋 Description

• Develop, assess, and coordinate holistic care management activities • Evaluate member needs and available resources, and recommend or facilitate the best-outcome plan • Develop ongoing long-term care plans/service plans • Identify providers, specialists, and community resources • Coordinate members/families/caregivers with care provider teams • Provide resource support for employment, housing, participant direction, independent living, justice, and foster care needs • Monitor care plans, member status, and outcomes • Interact with long-term care healthcare providers and partners • Collect, document, and maintain member information and care management activities for regulatory compliance • Perform home and/or other site visits as needed • Educate members and families/caregivers on procedures, provider instructions, service options, referrals, and healthcare benefits • Provide leadership feedback on quality-of-care and service-delivery improvements • Comply with all policies and standards • Perform other duties as assigned

🎯 Requirements

• Must reside in Sedgwick County, Kansas • Bachelor's degree and 2–4 years of related experience; or bachelor's degree in health, human, social work or education services with one or more years of qualifying experience; or high school degree/equivalent and three years of qualifying case management experience • Case management experience • Computer proficiency • Proficiency in Microsoft applications • Strong communication and customer service skills • Excellent attention to detail • Assessment, organizational, management, and time management skills • Ability to work independently, take initiative, and manage responsibilities effectively • Standard availability Monday through Friday, 8:00 AM to 5:00 PM
