📋 Description

• Develops, assesses, and coordinates holistic care management activities for long-term care members • Develops or assists with personalized service care plans/service plans • Evaluates member needs and available resources and recommends or facilitates appropriate care plans • Identifies providers, specialists, and community resources for long-term care • Coordinates between members/families/caregivers and care provider teams • Provides resource support for employment, housing, participant direction, independent living, justice, and foster care needs • Monitors care plans, member status, and outcomes and recommends updates based on member needs • Interacts with long-term care healthcare providers and partners • Collects, documents, and maintains member information and care management activities for regulatory compliance • Performs home and other site visits to assess member needs and collaborate with healthcare providers and partners • Educates members and families/caregivers on procedures, provider instructions, service options, referrals, and healthcare benefits • Provides leadership feedback on opportunities to improve quality of care and cost-effective service delivery • Performs other duties as assigned

🎯 Requirements

• Bachelor's degree and 2–4 years of related experience, or equivalent experience acquired through applicable knowledge, duties, scope and skill • Candidate must reside in Pennsylvania • Previous assessment experience strongly preferred • Experience with Community Health Choices (CHC) strongly preferred • Ability to travel approximately 50% to nursing facilities • Ability to perform home and/or other site visits • Compliance with all policies and standards
