📋 Description

• Coordinate services and care for patients through coaching, strategies, and coordination of health and wellness resources • Identify individuals who may benefit from additional support and connect them with behavioral health resources • Promote integrated, person-centered care between medical and behavioral health services • Help patients navigate the health care system through coaching, education, and support • Coordinate behavioral and primary care needs and develop strategies to remove barriers to wellness • Refer, advocate, and link clients and families to additional services • Use behavioral management techniques, crisis intervention, active listening, and motivational interviewing • Provide supportive and educational case management services to develop daily living and coping skills • Develop, implement, evaluate, and update treatment plans with client and family input • Enter timely written documentation into the Electronic Medical Record • Connect with clients face to face, in the office and community, via telehealth, and by phone • Demonstrate ethical practice with patients and community partners • Attend weekly staffing and team meetings and complete assessments • Assist clients with developing natural support systems • Attend and participate in clinical supervision

🎯 Requirements

• Bachelor’s Degree in psychology, social work or related field required • Experience with Electronic Health Record preferred • Valid driver’s license in good standing required • Complete all required agency trainings to maintain 100% compliance • Maintain departmental productivity expectations • Attend and participate in clinical supervision • Ability to use behavioral management techniques, crisis intervention, active listening and motivational interviewing • Ability to develop, implement, evaluate, and update treatment plans • Ability to document services in an Electronic Medical Record • Ability to work face to face in the office and community, in patient homes, via telehealth, and by phone
