📋 Description

• Deliver telephonic, patient-centered care in a structured call-center environment throughout the shift • Conduct comprehensive telephonic assessments and evaluations of members • Provide medication therapy management and education on administration, side effects, and adherence • Develop and implement individualized care plans • Monitor patient progress through ongoing assessments and follow-up interactions • Collaborate within established protocols using advanced telecommunication systems • Perform scheduled inbound and outbound tasks • Work assigned weekday, Friday rotation, Saturday rotation, and holiday rotation shifts • Work overtime as needed • Attend occasional Humana office training or meetings

🎯 Requirements

• Bachelor of Science in Nursing (BSN) — required and must be completed • 3–5 years of clinical acute care experience as an RN • Licensed Registered Nurse (RN) in the appropriate state with no disciplinary action • Must live in Ohio, Kentucky, Florida, Arizona, or Texas • Proficiency with Word, Excel, and Outlook • Managed care experience • Passion for improving consumer experiences • Schedule adherence • Experience with care management, adult patient education, and auto dialer — preferred • Bilingual English/Spanish — preferred • Minimum home internet speed of 25 Mbps download and 10 Mbps upload • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
