📋 Description

• Integrate programs developed to improve overall health outcomes, focusing on prescription drugs • Oversee medication therapy and strategy for comprehensive medication reviews • Optimize prescription drug use for patients taking multiple medications • Promote health awareness through Rx education and targeted quarterly campaigns • Place outbound calls to confirm patients are taking drugs and provide counseling • Apply professional concepts, standards, regulations, strategies, and operating standards • Make decisions regarding own work approach and priorities while following direction • Travel occasionally to Humana offices for training or meetings if required • Document and manage information using EHR systems

🎯 Requirements

• An active, unrestricted LPN license • One (1) or more years' experience working as an LPN • Experience working with Electronic Health Record (EHR) systems for accurate documentation, data management, and workflow optimization • Demonstrated ability to solve complex problems and encourage collaborative problem-solving across and within teams • Proven ability to work independently while contributing effectively to team success • Monday through Friday availability, with shifts assigned between 8:00 AM and 8:00 PM Eastern Time Zone (ET) • Minimum internet speed of 25 Mbps download and 10 Mbps upload • Ability to work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information • Preferred: Mail Order Pharmacy experience • Preferred: One (1) year or more years of experience working as an LPN in a specialty pharmacy setting • Preferred: LPN Compact license
