📋 Description

• Provide care management to a group of health plan members • Create Person-Centered Service Plans (PCSP) • Coordinate services and document care according to health plan standards • Assess, plan, facilitate, and advocate for member support and services through an interdisciplinary approach • Implement quality and cost-effective plans of care using available resources and UAS-NY assessments • Collaborate with members, families, healthcare teams, medical providers, and community-based programs • Oversee a caseload of managed care members • Complete required regulatory documentation clearly, concisely, and timely • Coordinate health plan support staff, including LPNs, Community Health Workers, and Care Coordination • Provide guidance and education to support staff • Participate in departmental, staff, and training meetings • Comply with state and federal privacy regulations, including HIPAA • Communicate and collaborate with members, families, health plan staff, and providers • Act as a liaison and member advocate • Demonstrate cultural competence and address members' cultural health beliefs, language needs, and support systems • Perform other duties as assigned

🎯 Requirements

• License and current registration as a Registered Professional Nurse in New York State • Associate degree required • Minimum of one year of Nursing Experience required • Long-term care and/or managed care experience a plus • Knowledge of homecare, Medicare, and Medicaid a plus • Care Management Certification a plus • Bilingual skills a plus • Ability to work independently and prioritize workload • Computer literacy and working knowledge of basic computer skills and Microsoft Office programs • Strong communication skills, both person-to-person, virtually, and by phone • Valid NYS driver's license and ability to drive in all types of weather • Ability to learn and routinely utilize the organization's software systems • Ability to perform duties involving exposure or potential exposure to blood, body fluids, or tissues • Ability to meet physical requirements, including lifting up to 20 pounds frequently and up to 50 pounds occasionally
