📋 Description

• Conduct thorough remote patient assessments via phone or video conferencing • Develop, implement, and update individualized care plans with patients and healthcare teams • Monitor patient progress and treatment-plan adherence • Provide education and guidance to promote self-care and disease management • Coordinate with physicians, specialists, and other healthcare professionals • Document patient interactions, clinical findings, and care activities accurately in electronic health records • Respond to patient inquiries and urgent health concerns, escalating issues as necessary • Participate in quality improvement initiatives and continuing education • Maintain clinical competencies and enhance patient care services

🎯 Requirements

• Current and unrestricted Registered Nurse (RN) license in the United States • License in a compact state, with preference for additional licenses in California and Illinois • Minimum of two years of clinical nursing experience • Proficiency with electronic health record (EHR) systems and telehealth technology platforms • Strong communication skills for engaging with patients and interdisciplinary teams remotely • Ability to work independently in a remote environment while managing multiple patient cases efficiently • Certification in Case Management (CCM) or Telehealth Nursing (CTN) highly desirable • Experience in a remote or virtual care setting preferred • Background managing complex chronic conditions such as diabetes, heart failure, or COPD preferred • Familiarity with healthcare regulatory requirements and telehealth quality standards preferred • Additional training or education in patient education, motivational interviewing, or behavioral health preferred
