Nadia Care is a membership-based maternal support service that provides community-centered, in-person and online care for people during pregnancy and after childbirth. They provide expert maternity coaching, coordinate a team to support feeding, nutrition, labor, and recovery, and connect members to local resources (housing, rides, other supports). Nadia Care works alongside a member's clinical providers to fill gaps in support and scale maternal care services through partnerships (noted as an ACAP Preferred Vendor).

📋 Description

• Serve as a clinical link between care and community-based support for new mothers and their babies • Provide guidance throughout pregnancy and the postpartum period • Conduct telemedicine visits at set points in the postpartum period • Identify unmanaged clinical needs and educate members on complex clinical topics • Assess Urgent Maternal Warning Signs and high blood pressure values • Help members navigate to the most appropriate care setting • Review blood pressure and Edinburgh Postnatal Depression Scale (EPDS) values • Follow up on irregular values that have not already been escalated • Liaise clinically with members’ obstetrics providers or primary care physicians • Document visit summaries and escalate unmanaged clinical needs to the Care team • Train Maternity Navigators on prenatal and postpartum topics • Bring community insights into Nadia Care program development • Foster collaboration between Maternity Navigators and clinicians through case discussions, knowledge sharing, and care team huddles • Report to the Senior Manager of Care Delivery

🎯 Requirements

• Active compact state Registered Nurse (RN) license in TN or IN, with interest in obtaining licensure in other compact states • Minimum of 5 years of experience in prenatal and postpartum care in outpatient pediatrics, OB/GYN, maternal-fetal medicine, acute care, or a similar setting • Native or bilingual fluency in speaking, writing, and reading Spanish • Experience delivering clinical information, patient education, and medical guidance in Spanish • Exceptional interpersonal and communication skills • Ability to provide compassionate, empathetic, and culturally-informed care • Highly organized and solutions-oriented • Ability to adapt in a fast-paced, evolving environment • Comfortable establishing strong relationships with patients virtually • Tech-savvy or able to quickly learn systems for documenting care and communicating with members, such as AthenaOne, Spruce, and MacOS
