📋 Description

• Conduct thorough patient assessments via telehealth platforms to evaluate health status and identify care needs • Develop and implement individualized care plans with patients and healthcare teams • Monitor patient progress and treatment response, adjusting care plans as necessary • Provide education on disease management, medication adherence, and healthy lifestyle choices • Document patient interactions, assessments, and care plans in electronic health records • Coordinate with physicians, specialists, and other healthcare professionals • Respond to patient inquiries and concerns, offering remote guidance and support • Use telehealth technology for virtual visits and remote monitoring • Deliver virtual nursing care to manage chronic conditions and acute health issues • Contribute to reducing hospital readmissions and emergency visits through proactive intervention and timely communication

🎯 Requirements

• Current and valid Licensed Practical Nurse license in the United States • License in a compact state, with preference for additional licenses in California and Illinois • Minimum of one year of clinical nursing experience, preferably in outpatient, community health, or telehealth settings • Basic proficiency with electronic health record (EHR) systems and telehealth technology • Strong communication skills to interact with patients and healthcare team members remotely • Ability to work independently in a remote environment while managing time and priorities efficiently • Preferred: experience in remote patient monitoring or telehealth nursing • Preferred: certification in telehealth nursing or related specialty • Familiarity with chronic disease management programs such as diabetes, hypertension, or COPD • Preferred: advanced clinical training or certifications such as Certified Case Manager (CCM) or Certified Diabetes Educator (CDE) • Demonstrated ability to use data analytics tools to track patient outcomes and improve care delivery
