📋 Description

• Evaluate the suitability of pharmacotherapy for high-risk patients with chronic conditions using evidence-based medicine and clinical judgment • Execute clinical protocols in accordance with organizational and state regulatory requirements • Optimize drug coverage benefits through preferred formulary products, lower-cost alternatives, and home delivery pharmacy • Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care • Participate in multidisciplinary high-risk rounds and adjust treatment plans • Counsel patients and communicate complex medical information to patients and healthcare providers • Provide disease-state education on medication use and self-management • Engage in quality improvement initiatives focused on medication safety and efficacy • Perform post-discharge medication reconciliation for high-risk transition-of-care patients • Document clinical activity in the clinic EMR and other pharmacy or clinical management systems • Support CMS Stars and HEDIS quality measures • Maintain accurate documentation of clinical interventions and patient care activities • Conduct in-service education and training for healthcare staff • Develop and share educational materials related to pharmacotherapy • Stay current with pharmacotherapy advancements and clinical guidelines • Consult healthcare providers on complex medication management issues • Respond to Primary Care Organization requests within the pharmacist’s scope of practice • Implement targeted initiatives and priorities directed by pharmacy leadership according to best practices and regulatory standards

🎯 Requirements

• Active and unrestricted pharmacist license in the state of Florida • Willingness to become licensed in additional states for market support • Pharm.D. or other advanced pharmacy degree • Minimum five (5) years of experience as a practicing pharmacist • Experience working in a team environment and supporting common goals • Proficiency with MS Office programs including Word, Excel, PowerPoint, Teams, and Outlook • Ability to troubleshoot and resolve basic technical needs within a remote environment • Clinical residency in ambulatory care or similar discipline (preferred) • Board certification in geriatric pharmacy (preferred) • Experience working in an ambulatory care/primary practice setting (preferred) • Familiarity with electronic medical record (EMR) systems (preferred) • Experience managing drug therapy for patients with chronic conditions (preferred) • Ability to identify and implement innovative solutions for improved efficiencies and outcomes (preferred) • Multilingual, preferably English/Spanish (preferred) • Must reside within a 50-mile radius of Okeechobee or Lake Worth, Florida • Availability Monday through Friday, 8:00 a.m. to 5:00 p.m. Eastern • Ability to travel approximately 5%, typically once quarterly for market meetings • Home internet with minimum download speed of 25 Mbps and upload speed of 10 Mbps • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
