📋 Description

• Conduct Participant Intake & Assessment • Develop Individualized Service Plans (ISPs) • Provide Case Management & Support • Facilitate Resource Navigation • Track Progress & Maintain Documentation • Engage in Crisis Intervention • Collaborate with Partners • Advocate for Participants • Support Program Development

🎯 Requirements

• Prior experience in case management, social work, re-entry services, or a related field • Deeply committed to the dignity, rights, and empowerment of justice-involved individuals • Highly organized and able to manage multiple caseloads while maintaining empathy and professionalism • Strong verbal and written communication skills • Familiar with Washington, D.C.-based resources and systems relevant to returning citizens • Trauma-informed and culturally responsive in your approach to client engagement • Comfortable using case management databases and common software (Microsoft Office, Google Suite) • Bilingual candidates are encouraged to apply
