📋 Description

• Serve as the first point of contact for Hope PACE or Adult Day Health Center • Support the assigned team in daily functions • Triage phone calls and route messages appropriately • Perform administrative and clerical duties, including filing and office supply management • Maintain client schedules and anticipate future needs • Enter and update patient care, demographic, timesheet, careplan, report, referral, admission, and statistical data • Generate and distribute accurate reports and information changes • Communicate and collaborate with the interdisciplinary team in patient and family care • Coordinate and confirm participant appointments with outside providers • Provide internal and external customer service • Record IDT minutes, update agendas, and track periodic assessments • Coordinate paperwork delivery with HIM and outside providers • Receive, verify, review, interpret, and collate PACE referrals • Support the PACE intake process and conduct information visits with potential enrollees • Participate in quality processes, interdisciplinary and management meetings, marketing activities, and special projects • Assist with care-plan development and coordination of 24-hour care delivery • Represent the company professionally and complete other assigned duties

🎯 Requirements

• High School Diploma • Must possess a valid State ID • Certification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs • At least 2 years prior secretarial experience required • Medical office and terminology experience required • Excellent customer service skills • MS Word and general computer knowledge required • Ability to quickly learn other computer programs • Consent to drug and/or alcohol testing after a conditional offer of employment • Ongoing compliance with the Drug-Free Workplace Policy • Eligibility verification through the Florida Care Provider Background Screening Clearinghouse after a conditional offer and ongoing eligibility
