📋 Description

• Respond to overnight and weekend calls regarding changing hospice patient needs • Answer questions and gather information from patients and caregivers • Assess patient needs and coordinate/direct hospice services and care by phone conference • Instruct patients and caregivers on appropriate nursing interventions • Evaluate intervention options and apply critical thinking to select appropriate care • Determine the need for nursing visits using the EMR and telephone-call information • Dispatch visit RNs or LPNs as appropriate • Complete, submit, and maintain accurate clinical notes • Record pain and symptom management changes and outcomes • Communicate patient needs and condition changes to physicians and obtain orders • Support patient self-sufficiency and comfort care • Counsel and cooperate with families, caregivers, and the hospice interdisciplinary group • Ensure equipment and necessary services are arranged • Maintain patient confidentiality and use protected health information appropriately • Participate in quality assessment and performance improvement activities • Perform other duties as assigned

🎯 Requirements

• Current Registered Nurse (RN) license in state(s) of practice • Possess or obtain an RN license for the 10 states served and/or a compact license before hire • Graduate of a professional school of nursing • Currently licensed, certified, or registered in the state(s) of practice • Ability to pass DHS background, consumer report, and drug test • If driving: valid state driver’s license, automobile in good working order, required automobile insurance, and acceptable driving record • Preferred 1+ years of recent hospice/home care experience • Preferred 2+ years of nursing experience • Ability to push/pull and lift/carry up to 30 pounds, occasionally up to 50 pounds • Ability to adapt to a variety of work conditions in a home setting • Compliance with HIPAA regulations and requirements
