📋 Description

• Assess and identify patient/family needs using the nursing process • Coordinate individualized Plans of Care with the Interdisciplinary Team (IDT) • Provide direct clinical, palliative, supportive, psychosocial, spiritual, cultural, physical, and biological care • Provide telephone reassurance and determine when in-person visits are needed • Coordinate and document patient/family contacts in the EMR • Complete initial and semi-annual assessments • Explain services and address patient/family questions and concerns • Initiate skilled nursing interventions, obtain physician orders, and complete required documentation • Represent the Company at assigned facilities and facilitate referrals across service lines • Communicate clinical information to the IDT using SBAR • Conduct home visits to assess safety, medication and nutritional compliance, DME compliance, and ability to live safely in the community • Educate patients, families, caregivers, and health professionals about disease progression, palliative interventions, caregiving, dying process, and safety • Report patient condition changes to IDT members and other health professionals • Evaluate hospice referrals and admissions with the IDT • Attend daily IDT collaboration meetings and present oral and written reports • Supervise care provided by Community Health Workers and Home Health Aides when requested • Report to a company-designated location and potentially stay overnight during emergencies • Perform other assigned duties

🎯 Requirements

• Current RN license in the state where the employee will be working • Minimum of one year of nursing experience • Hospice or hospital experience preferred • Employees working at PACE must complete Alzheimer’s Disease and Related Dementias Training through the Florida Department of Elder Affairs • Previous experience with an EMR/EHR system • Valid driver’s license and automobile insurance per Company policy • Reliable transportation to meet the visit schedule • Ability to use equipment with visual and auditory mechanisms • Ability to communicate effectively in English, verbally and in writing • Ability to visit participants in their homes for assessments • Ability to perform essential job functions and physical requirements, including lifting, bending, pushing/pulling, and kneeling, with or without reasonable accommodation • Active BLS for healthcare professionals from the American Heart Association or Red Cross • Satisfactory completion of competency requirements • Consent to drug and/or alcohol testing after a conditional offer • Ongoing compliance with the Drug-Free Workplace Policy • Eligibility verification through the Florida Care Provider Background Screening Clearinghouse for employees serving Florida affiliates
