📋 Description

• Provide diagnostic patient care primarily through virtual remote consultation via video conference or telephone • Serve as dispositional authority for after-hours and weekend clinical calls • Assess, diagnose, treat, and document patients’ physical and psychosocial health status • Assess acute and non-acute clinical problems through video conference or telephone • Perform and document physical evaluations and patient histories • Analyze patient-condition trends and develop, document, and implement patient management plans • Develop plans of care including health education, physician referrals, case management referrals, and patient/family counseling • Collect and analyze comprehensive and focused patient and family health data • Confer with PCPs and other medical providers to report health data and ensure guideline compliance • Use telemedicine to achieve optimal patient outcomes • Collaborate with on-call PCPs to support clinical outcomes • Implement protocols and evaluate progress toward expected outcomes • Enhance telehealth practices and systems using data demonstrating program effectiveness • Communicate through varied formats, tools, and technologies to deliver care across the continuum • Provide safe, effective, financially responsible services and extraordinary customer service • Participate in clinical rounds, conferences, risk and quality management programs, and relevant meetings • Adhere to departmental standards, regulatory requirements, quality-care policies, and procedures • Practice under a written or electronic practice agreement • Participate in developing telehealth/telemedicine policies, procedures, and protocols • Initiate and participate in quality improvement activities • Participate in committees supporting organizational growth • Provide feedback regarding others’ practice to improve patient care • Coordinate services with other programs • Travel to Georgia quarterly or Tennessee bi-annually for required onsite observation and medical record review when supporting patients in those states • Perform other duties as assigned

🎯 Requirements

• Bachelor’s degree in Nursing (BSN) • Graduate of a school of nursing for Advanced Practice Nursing • Master’s degree in Nursing • Board certification by AANP or ANCC • Basic Life Support (BLS) certification from the American Heart Association or American Red Cross upon hire • Current DEA number for Schedule II-V controlled substances may be required based on state of practice • Multi-state licensure including Florida and at least three licenses among VA, GA, MI, MO, OH, PA, TN, TX, IL, and LA • Minimum 4 years of acute/primary care clinical work experience as a Nurse Practitioner • 2 years of telehealth work experience highly preferred • Expert knowledge of core job-related functions, practices, processes, procedures, techniques, and methods • Knowledge of medical practices to function independently as a certified practitioner and collaborate with physicians and other medical providers • Demonstrated record of achieving clinical performance metrics • Technical capability to conduct telemedicine visits in accordance with state and federal regulations • Excellent clinical judgment and problem-solving ability • Ability to prioritize and work under pressure • Ability to provide constructive feedback • Professional communication and collaboration with physicians, patients, and team members • Ability to operate effectively with a multidisciplinary team • Proficiency in Microsoft Office Suite, including Excel, Word, PowerPoint, and Outlook • Ability and willingness to travel up to 10% for meetings and trainings • Availability for evenings/overnights and/or weekends depending on assigned schedule • Minimum requirement to work four holidays per calendar year • Spoken and written fluency in English • Ability and willingness to obtain independent/autonomous practice as an NP in applicable states
