📋 Description

• Deliver holistic, evidence-based remote patient care through assessment, diagnosis, treatment, health-problem management, health counseling, and disposition planning • Provide education and treatment for pregnancy prevention, STI prevention, safer sex practices, contraceptive care, and medication management • Evaluate primary care, acute, chronically ill, and transitional care patients aged 18 months and above • Educate patients on health maintenance and respond to patient care inquiries • Document all patient care in an EHR according to MinuteClinic policies and procedures • Coordinate patient care with internal and external colleagues, including the broader patient-centered medical home • Work within a patient care team including Providers, Collaborative Physicians, paraprofessionals, Pharmacists, and other healthcare team members • Work independently, prioritize and solve problems, take initiative, and advocate for patients and the practice

🎯 Requirements

• Current unrestricted licensure in FL, MA, and OH stated for qualified candidates; candidates must currently be licensed in D.C., DE, FL, KS, KY, MA, MD, ME, NH, OH, RI, or VT • Willingness and ability to obtain additional licensure if hired • Minimum of two years of medically relevant experience or equivalent • Effective verbal, written, and electronic communication skills • Outstanding organizational skills and ability to multitask • Initiative, problem-solving ability, adaptability, and flexibility • Ability to work without direct supervision and practice autonomously • Proficiency with information management and technology • Capacity to collaborate with professional colleagues to provide quality care • Depending on the market, proficiency in speaking and writing additional languages, including Spanish, may be required • Completion of a Master’s Degree-level Family Nurse Practitioner program • Current National Board Certification • State of Employment license to practice in the Advanced Practice Nurse role • Minimum 20 hours of availability per week • Ability to meet remote-care requirements, including broadband connectivity, a quiet setting with neutral background, patient privacy, and computer/phone operation
