📋 Description

• Conduct comprehensive telehealth visits for patients with obesity, diabetes, and cardiometabolic conditions • Initiate and manage advanced therapeutics, including GLP-1s, dual incretin therapies, oral weight management agents, SGLT2 inhibitors, and PCSK9 inhibitors • Monitor laboratory results, adjust medications, and ensure safe, effective treatment plans • Educate and empower patients on lifestyle modification, disease management, and medication adherence • Collaborate with physicians, pharmacists, dietitians, and behavioral health specialists • Support patients with medication access, including prior authorizations and adherence strategies • Utilize analytics and patient data to track outcomes and optimize care plans • Contribute to program development, workflow optimization, and quality improvement initiatives

🎯 Requirements

• Current Massachusetts NP or PA licensure and Master’s degree • Certification from an accredited Nurse Practitioner or Physician Assistant program • BLS and ACLS certification • Minimum of 2+ years of experience in an APP role managing chronic conditions, including obesity, diabetes, and cardiometabolic disease • Experience prescribing and managing advanced therapeutics, including GLP-1s, SGLT2 inhibitors, PCSK9 inhibitors, and weight management medications • Comfort working in a fully virtual care environment • Strong clinical judgment, communication, and critical thinking skills • Ability to work independently and collaboratively in a multidisciplinary team • Preferred: Experience with Epic EMR • Preferred: Experience in telehealth or digital health environments • Preferred: Interest in population health, preventive care, and value-based care models • Candidates must reside in one of the following approved states: Arizona, Connecticut, Florida, Massachusetts, Maine, Maryland, Michigan, New Jersey, New Hampshire, North Carolina, Ohio, Pennsylvania, Rhode Island, Texas, or Wisconsin
