📋 Description

• Provide primary and urgent health care via telephone and tele-video to patients receiving home and community-based services • Develop and implement clinical plans of care for adult patients with chronic and complex conditions • Gather patient history, physical examination information, and diagnostics as needed • Develop and implement treatment plans aligned with patient goals and current conditions • Identify and close gaps in care • Address patients’ and families’ physical and psychosocial needs with interdisciplinary team support • Educate patients and families on medication usage, side effects, illness progression, diet and nutrition, adherence, and crisis prevention • Coordinate care and resources with company physicians, other physicians, medical providers, patients, and families • Maintain patient medical records and medical documentation • Participate in required continuing education • Prescribe medication as permitted by state prescribing authority • Collaborate to develop complex plans of care consistent with patient health status, goals, and values

🎯 Requirements

• Master of Science in Nursing (MS in Nursing) • Active national Nurse Practitioner certification • Valid, current, active, unrestricted Family or Adult Nurse Practitioner license in Massachusetts • Valid, current, active RN license • 2+ years of experience managing complex care cases • Experience working with Electronic Medical Records (EMR) • Monday–Friday availability from 8:00 am to 5:00 pm local time • On-call availability 2–3 nights every 4–6 weeks and holiday coverage • Registered with Medicaid preferred • DEA registration or eligibility preferred • Experience managing complex care cases for developmental disabilities and chronically ill patients strongly preferred • Ability to provide care in accordance with state regulations and company health standards and policies • COVID-19 and Influenza vaccination required for certain patient/member-facing roles unless an acceptable explanation is provided
