📋 Description

• Provide diagnostic patient care primarily through virtual remote consultation by video conference or telephone • Provide on-call coverage and serve as the dispositional authority for after-hours and weekend clinical calls • Assess, diagnose, treat, and document geriatric patients’ physical and psychosocial health status • Work a minimum of 72 weekend hours every three months and at least 160 hours per year • Work as needed, including nights, weekends, and holidays • Pick up shifts covering PTO, leave of absence, and unexpected absences • Conduct virtual assessments of acute and non-acute clinical problems • Perform and document physical evaluations and patient histories • Analyze trends and develop, document, and implement patient management plans • Provide health education, physician referrals, case management referrals, and patient/family counseling • Evaluate needs for immediate nursing intervention, consultation, or referral • Collect and analyze comprehensive and focused patient and family health data • Confer with PCPs and other medical providers regarding health data and guideline compliance • Counsel patients and families on health outcomes and positive health habits • Lead efforts to achieve optimal patient outcomes through telemedicine • Collaborate with on-call PCPs and implement protocols to achieve expected outcomes • Document assessments, interventions, and progress in an easy-to-understand and translatable format • Enhance telehealth practices and systems using program effectiveness data • Use varied communication formats, tools, and technologies to deliver care • Participate in clinical rounds, conferences, risk and quality management programs, and relevant meetings • Adhere to departmental standards, regulatory requirements, quality-care policies, and practice agreements • Participate with physicians in developing telehealth policies, procedures, and protocols • Perform other duties assigned by management

🎯 Requirements

• Bachelor’s degree in Nursing (BSN) and graduate of a school of nursing for Advanced Practice Nursing with certification in area of specialty required • Master’s degree in Nursing required • Valid, active Registered Nurse license required • Board certification by AANP or ANCC required for NPs • Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within first 90 days of employment • Current DEA number for Schedule II-V controlled substances may be required based on state of practice • Multi-state licensure including FL, VA, and at least two licenses among GA, KS, MI, MO, NC, OH, PA, SC, TN, TX, IL, KY, and LA • Minimum of 3 years’ acute/primary care clinical work experience required • Minimum of 2 years’ telehealth work experience preferred • Expert business acuity and knowledge of job-related functions, practices, processes, procedures, techniques, and methods • Knowledge of medical practices to function independently as a certified practitioner and collaborate with physicians and other providers • Demonstrated achievement of clinical performance metrics • Technical capability to conduct telemedicine visits in accordance with state and federal regulations • Excellent clinical judgment and problem-solving ability • Ability to prioritize and work under pressure • Ability to communicate and collaborate professionally with physicians, patients, and team members • Ability to work effectively with a multidisciplinary team • Ability and willingness to travel locally within south Florida for meetings and trainings up to 10% of the time • Availability for evenings, overnights, weekends, and at least three holidays per calendar year • Proficiency in Microsoft Office Suite, including Excel, Word, PowerPoint, and Outlook • Spoken and written fluency in English; bilingual Spanish/Creole is a plus
