📋 Description

• Virtually connect with members eligible for Cityblock services and engage them around their needs • Conduct telephonic/virtual outreach and engagement • Use Commons data to learn about potential members and explain outreach processes • Review demographic information and document accurate information and emergency contacts in Commons • Collect verbal and written consent • Initiate intake and hand off members to care teams • Communicate urgent needs and pertinent information to care teams • Track tasks, monitor performance, and communicate with care teams, members, and family members in Commons • Provide feedback to product development and support onboarding as a future super-user

🎯 Requirements

• 1 year in a similar role • Ability to provide telephonic/virtual outreach and engagement services • Ability to use Commons to collect data, organize information, track tasks, and monitor performance • Ability to review and document demographic information and emergency contacts • Ability to collect verbal and written consent • Ability to initiate intake processes and handoffs to care teams
